Further, you agree and acknowledge that we have the right to immediately deposit any check you deliver in connection with a balance owed by you as set forth on the Portal. Privacy Laws and Privacy Practices

You agree to indemnify and hold the Operator, all medical providers whose information is provided in this Portal, and each of their respective Representatives harmless from any claims, demands, losses, damages, fees, or penalties, and the cost to investigate and defend same including reasonable attorneys’ fees, made by you, any federal or state agency, or any third party due to or arising out of the breach of these Terms or a violation of the Privacy Laws by you or anyone accessing this Portal on your behalf.
Right to Change the Terms and Conditions
The Terms may be updated by us from time to time without notice to you. Any changes to the Terms will be effective immediately upon posting of the charged terms and conditions on the Portal. You agree to review the Terms periodically, and your use of the Portal following any such change constitutes your agreement to follow and be bound by the terms and conditions as changed.
The Terms constitute the entire agreement between you and the Operator and governs your use of the Portal. The Terms and the relationship between you and the Operator shall be governed by the laws of the State of Indiana without regard to its conflict of law provisions. The failure of the Operator to exercise or enforce any right or provision of the Terms shall not constitute a waiver of such right or provision. Class Action Waiver
THERE WILL BE NO RIGHT OR AUTHORITY FOR ANY DISPUTE TO BE BROUGHT, HEARD, OR ARBITRATED AS A CLASS ACTION (INCLUDING WITHOUT LIMITATION OPT OUT CLASS ACTIONS OR OPT IN COLLECTIE CLASS ACTIONS), OR IN A REPRESENTATIVE OR PRIVATE ATTORNEY GENERAL CAPACITY ON BEHALF OF A CLASS OF PERSONS OR THE GENERAL PUBLIC. ALL DISPUTES SHALL BE RESOLVED ON AN INDIVIDUAL BASIS ONLY.

Pacific Surgical Center may be listed on your credit/debit/HSA card statement for payments made through this website. If you have questions, please contact out billing department at 360-442-7902.
Pacific Surgical Center may issue a requested refund on a credit/debit/HSA card payment for the following reasons:
- An overpayment on your account, or due to an adjustment being posted to the your account
- Customer transaction entry error, such as a payment being made for more than intended ($500.00 instead of $50.00)
- Duplicate Payment
We will do our best to resolve your request within 24 hours (1 business day) of the request being submitted, unless that request is received in the afternoon, on a Friday or holiday.

Pacific Surgical Center will not disclose any personal information obtained through the Pacific Surgical Center site for any third-party marketing purposes.
